The Ministry of Education and Training said that in the national high school exam, there are two forms for candidates to register, including the exam registration form and the high school graduation recognition registration form. Each form is divided into 4 groups of information and candidates are responsible for filling in completely and accurately.

The front of the exam registration form includes 4 groups of information: Personal information; Exam registration information; Information for graduation recognition; Information for priority consideration in university and college admissions. The back of the form is instructions for candidates on how to fill out the exam registration form.
